Group Home Services

We believe that our clients have the right to be integrated back into our community with as much normalcy as possible. Because our agency recognizes the critical need for community reintegration, SCCS has expanded our core services to provide group home services to the Intellectually Disabled population. Our staff members are dedicated to improving the lives of each and every individual with our focus on 24-hour quality care.

* Licensed Provider of Group Home Services for the Intellectually Disabled

  • Individual must be 18 or older
  • Each resident will receive cognitive stimulation in a supportive therapeutic environment while strengthening their community support.
  • All Funding for Second Chances Group Homes are Secured through Medicaid, and Private Insurance.

Admission Criteria

All individuals regardless of race, religion, color, sex, age, creed, or national origin, regardless of physical and metal capacity, shall have the opportunity to achieve their maximum potential while participation in our group home services that SCCS has to offer.

  • Client must have an Intellectual disabled diagnosis
  • Must be over 18 year old
  • And lack daily living skills such as:

    • Self Direction
    • Health and Safety
    • Educational Skills
    • Self Care
    • Social Skills

Method of Payment

The services provided by SCCS can be funded by long-term insurance, Medicaid Support, Private Pay, or ID-Waiver.

Health Related Services

  • Assisting with administration of medicine
  • Monitoring vital signs
  • Supervision
  • Incontinence Care

Respite Care Activities of Daily Living

  • Meal Preparation
  • Full or Partial Bath Assistance
  • Toileting
  • Assistance with morning wake-up and evening tuck-in

Household Maintenance

  • Light cleaning
  • Laundry and Ironing Support
  • Shopping
  • Changing bed linens
  • Assistance with coordinating home services
